I can only say that sometimes I have see MAC OS X strange behaviors if open close files fast (from code).
Usage of another FolderItem (for Xojo) could help.

What exactly operation produce error you show?
Is this on user machines or you can get it self?

I don't know where this occurs in terms of the code and database access, it's just reports I get from users. It's not common, but I've had 3 or 4 such reports in the last 4-6 weeks. If I can figure out a pattern I'll let you know. I'd be surprised if no one else is running into this.
